The Community Collaborative Wellness Kit includes the following components:

  • A Rationale for the Community Collaborative Wellness Tool
    This rationale defines key elements of reform, the stages of collaborative change and key cross-cutting themes.

  • Answers to Questions on the Community Collaborative Wellness Tool
    The tool uses questions as a device to inquire into the work of community collaboratives. Here TWC answers its own questions providing a rationale for each question and pathways toward strategies to address them within the collaborative.

  • A Bibliography of Resource Materials
    Documents related to the elements of reform and cross-cutting themes.

  • A Directory of National Resource Organizations
    Organizations that can provide information, technical assistance and other support to community collaboratives.

  • A Training Program
    A program to prepare facilitators to use the TWC Community Collaborative Wellness Kit. (Coming soon.)

  • A Collaborative Assessment Process
    This process enables community collaboratives to determine their progress toward systems reform and chart a more strategic course of action. (Coming soon.)

Using the Community Collaborative Wellness Kit
The TWC Wellness Kit is designed to be used with individual community collaboratives in their own settings, or with teams from collaboratives in leadership development sessions involving other collaboratives. TWC provides facilitators to conduct these events. Costs for individual sessions depend on the size of the groups and the extent to which time is required to modify existing TWC leadership development designs.
